The escalating cost of healthcare in Massachusetts is a pressing issue, and Blue Cross CEO Sarah Iselin is stepping up to the plate. At the recent 2025 annual meeting of the Retailers Association of Massachusetts, Iselin, as the head of the state's largest health insurer, made a bold commitment: to seriously tackle rising healthcare expenses. This echoes Governor Maura Healey's earlier promise to address the same problem, which garnered significant applause from retailers a year prior.
Iselin is wielding her power to curb costs, putting hospital systems on notice. She's holding them to the state's medical inflation cap of 3.6 percent, a stance that has already sparked conflict.
One major point of contention? The renewal of a substantial contract with the UMass Memorial Health system, affecting nearly 200,000 members. While an agreement was eventually reached, the tension highlights the challenges ahead.
Iselin emphasized her commitment to the 3.6 percent benchmark, stating, "We’re taking it seriously." Hospital operators, however, often argue this cap is too restrictive, especially considering the soaring labor costs, particularly for nurses, since the COVID-19 pandemic.
But here's where it gets controversial... Iselin acknowledges the difficult balancing act, recognizing that people simply can't afford to pay more. She cited a report placing Massachusetts at 48th in the nation for healthcare costs borne by employers.
"That’s not the direction you want to go in," Iselin stated, adding that she doesn't foresee a slowdown in the coming years.
Jon Hurst, the RAM president, expressed disappointment that state lawmakers didn't prioritize cost control when they initially implemented the Romneycare health care reforms in 2006. The Health Policy Commission, created to address cost control, came later, in 2012.
And this is the part most people miss... Hurst also voiced concern over the recent reorganization of the HPC, which saw the absence of an employer representative. He questioned whether the state is truly committed to tackling healthcare costs. Iselin's response was clear: "We’re serious about it."
She concluded on an optimistic note, suggesting that policymakers can find innovative solutions. She believes that all parties involved, including health plans, will need to make concessions.
